Our programmes are informed by patient safety learning, including coroners’ Prevention of Future Deaths reports, the Patient Safety Incident Response Framework (PSIRF), and national investigations such as the HSSIB reports into delayed diagnosis of heart attacks associated with ECG interpretation. We help healthcare professionals translate these lessons into safer clinical practice.
